How Do I Create An Account? When you complete an Airway Labs Prescription form and send it with the patient's models or Poly Vinyl impressions, we will call to inform you that we have received your case and obtain your contact information to create an account for you. Your account number will appear on your invoice. When we return the appliance to you, we will include our lab slips and a UPS label (for future cases or returns). What Do I Need to Send? Send models or Poly Vinyl impressions of the patient’s teeth. The models do not need to be mounted on an articulator. It is very important that the models are accurate and without air bubbles. If they are not accurate, it will be more difficult and time-consuming to seat the appliance. There is a space on the lab slip to write special instructions if needed. Important: A bite registration is not needed for the TAP I, however, we do require a 60% protrusive bite registration for the TAP 3 and the dreamTAP.
How Should I Send the Models? Airway Labs provides pre-paid UPS shipping labels. Please contact our office and a label will be faxed or emailed to you. Please take the package to a UPS store or to a UPS drop off box or call 1-800-PickUPS to schedule a pickup (additional charges may apply).

Where Do I Send the Models? If you do not wish to use our pre-paid UPS label, please send the case to the following address: Airway Labs 4300 Alpha Rd., Suite 115 Farmers Branch, Texas 75244
What is the Lab Turn-Around Time? Once we receive the models from you, there is a five-business day lab turn-around. The appliance is shipped back to you via UPS Ground shipping. You can also choose to have the case “rushed” for an additional $100. In the case of a rush, your appliance will ship UPS Overnight 24 hours from the time the lab receives your case.
Will the Models Be Returned? Yes. Please be aware that damage may occur during fabrication but every effort is made to preserve the casts’ originality. If you would like a set of intact models, please note for the models to be duplicated on the lab slip.
What Are Your Office Hours? Airway Labs is open Monday through Thursday from 8:00 am to 4:00pm, and Fridays from 8:00 to 3:00 pm (Central Standard Time).
When is Payment Due? An invoice is mailed shortly after you have received the case. You may pay from this invoice or monthly statement upon receipt. Payment is due 30 days from the invoice date.
When Should I Schedule the Patient to Deliver the Appliance? You should schedule your patient at least 2.5 to 3 weeks from outbound shipping date. This allows time for the appliance to arrive to Airway Labs, the 5-day in-lab fabrication time period, and UPS Ground shipping to and from your office.
